The cover scene (or anything remotely like it) does not appear in the issue; Eddie Berganza has said (through Matt Idelson on a column at SupermanHomePage) that the cover was symbolic, not literal, representing the culmination of the whole "Kara was sent to Earth to kill Superman" story.

Pariah confronts Supergirl, who’s still in space. He tells her he is drawn to catastrophic events, and he was brought back from death by Dark Angel to generate the illusion of what could happen with the phantoms on Earth. He tells her that she will be the key to future events, including an upcoming great disaster. He then leaves her with hope. She finally returns to Earth, and discusses things with Boomer, Grace, Cassie, Power Girl, Streaky, and Sarah. She even gets flowers from Power Boy, but still hates him. She finally gets the courage to have a talk with Clark.
